WIfI Classification — Limb Threat & Amputation Risk

Enter the wound, perfusion, and infection details below to compute WIfI grades (0–3) and the corresponding clinical stage (1–4) for 1-year amputation risk.

1. Wound (W) — choose the best descriptor

Grade 0: No ulcer, no gangrene.

Grade 1: Small, shallow ulcer on distal leg or foot; no exposed bone; no gangrene (minor tissue loss).

Grade 2: Deeper ulcer with exposed bone/joint/tendon or gangrene limited to digits (major tissue loss but not extensive).

Grade 3: Extensive / deep ulcer (forefoot/midfoot/heel with calcaneal involvement) or extensive gangrene (requires complex reconstruction/amputation).

2. Ischemia (I) — choose which perfusion test you have and select the matching range

Notes: Ranges follow WIfI/ESVS commonly used thresholds; select the range that matches your measurement.

3. Foot Infection (fI)

Grade 0: No symptoms or signs of infection.

Grade 1 (Mild): Local infection involving only skin/subcutaneous tissue (no deeper structure, no systemic signs).

Grade 2 (Moderate): Local infection spreading to deeper tissues (abscess, osteomyelitis, septic arthritis, fasciitis) OR erythema >2 cm < systemic signs.

Grade 3 (Severe): Local infection with systemic inflammatory response (SIRS) or other systemic signs (eg. temp >38 °C, HR>90, RR>20, WBC>12k or <4k).
